About
The proportion of our senior population is steadily on the rise, and there is an increasing need for and demand on community services, home care services, and family caregivers to provide a degree of support that ensures older adults can remain safe and well in their homes for as long as possible.
As people age we can predict that at some point their health and social situations are going to change. Increasingly, there is a need to address the issue of vulnerable older adults who live at risk in the community. Many have significant cognitive, mental health, and physical problems yet may not seek assistance. Support and intervention requires an interdisciplinary approach to care (Culo, S., 2011).
Who are we?
Guelph Wellington Seniors at Risk Network is a collaborative of cross-sectoral service providers working to ensure there is a coordinated community response to supporting older adults living at risk in Guelph Wellington.
Guiding Principles:
- Client autonomy is upheld – the client has the right to determine/direct their care to the full extent of their ability. Includes the risk to live at risk unless deemed incapable or there is imminent danger
- Client safety – right to live in a safe environment free from abuse
- Client dignity and respect – the right to have their privacy, dignity, interests, lifestyle, personal choices, cultural and religious beliefs respected and valued
- Client access to information – the client has the right to access the information necessary to make meaningful and informed choices
- Client confidentiality – the client has the right to confidentiality and privacy
- Interventions and service delivery to the older adult at risk should be the least intrusive of the client’s rights, abilities and personal liberties, and least disruptive to their lifestyle
- A non-judgmental and inclusive approach will be taken to support the older adult at all times, including the need to be aware of, and sensitive to diverse needs and considerations
- Care partners/caregivers and client’s social network are key partners
The Guelph Wellington Seniors at Risk Network meets regularly as a consultation team with the goals of:

Increasing awareness & understanding of complex needs of older adults in the community

Discussing and providing consultation on specific cases of seniors at risk and elder abuse

Offering consultation in emergency situations between meetings to concerned community agencies

Coordinating the efforts of the various agencies dealing with seniors and build a better understanding and respect for all agencies involved

Decreasing challenges in responding to older adults at risk by developing appropriate resources, implementing preventative strategies and/or identifying/intervening in cases as early as possible
Minimizing and Managing Risk – A Harm Reduction Approach
Please note that although we describe many facets of risk in this resource, note that one single factor does not imply risk and everyone, regardless of age, has the right to live in a manner of their own choosing.
The Guelph Wellington Seniors at Risk Network uses and promotes a harm reduction approach to living at risk whenever possible. Harm reduction means a focus on promoting and making the older adult’s situation safer and healthier while minimizing harm, rather than focusing on eliminating all possible risk.
